Why Cash-Pay, Out-of-Network Pelvic Floor Therapy Is Worth It

Let me say something that might surprise you: the fact that we're out-of-network isn't a drawback. It's the whole point.
I know, "cash-pay" can sound like a red flag. We're conditioned to believe that if insurance doesn't cover it, it must not be worth it, or worse, that we're being taken advantage of. But when it comes to pelvic floor therapy, the opposite is usually true. The in-network model wasn't built for the kind of care your body actually needs, and once you understand how it works behind the scenes, the reason so many pelvic floor specialists choose to practice this way starts to make a lot of sense.
So let's talk about what cash-pay, out-of-network care actually means, and why it might be exactly the kind of investment your healing deserves.
What "Cash-Pay, Out-of-Network" Actually Means
Being out-of-network simply means I don't bill your insurance company directly. You pay for your visit, and I provide you with a superbill, a detailed receipt with the diagnosis and treatment codes, that you can submit to your insurance for possible reimbursement if you have out-of-network benefits.
What this means for you is that your care is no longer dictated by an insurance company's rules. It's dictated by what your body actually needs.
The Problem With the In-Network Model
Here's what most people don't realize about in-network pelvic floor PT: insurance companies don't get a say in your diagnosis, but they absolutely get a say in how your treatment is delivered.
- Time. Insurance reimbursement rates are so low that many in-network clinics have to schedule 3-4 patients per hour just to stay afloat. That often means 15-20 minutes of actual one-on-one time with your therapist, with a tech or aide filling in the rest.
- Session limits. Insurance plans frequently cap the number of visits allowed per year, regardless of whether you're actually healed. Pelvic floor dysfunction is rarely a quick fix, and being cut off mid-progress is one of the most common reasons women fall through the cracks.
- Medical necessity requirements. Insurance only covers treatment for a diagnosed problem, not prevention, not optimization, not "let's make sure everything is functioning well before it becomes a problem." If you want proactive care, cash-pay is often your only option.
- Loss of continuity. In many in-network clinics, you're assigned whoever is available that day rather than building a relationship with one provider who knows your full history.
None of this reflects bad intentions. It reflects a reimbursement system that was never designed around the kind of hands-on, relationship-based, whole-body care that pelvic health actually requires.

And many of my patients are able to submit their superbill and get partial reimbursement through their out-of-network benefits or use an HSA/FSA to cover the cost. It's always worth checking what your specific plan allows.
