Let me say something that might surprise you, pelvic floor therapy is not just for pregnant women.

I know, it's the first thing most people think of. And yes, pelvic floor physical therapy is incredibly powerful during pregnancy and postpartum recovery. But if that's where the conversation ends, we're leaving so many women behind. Women who are living with chronic pelvic pain, bladder issues, painful periods, and intimacy concerns who have been told, sometimes for years, to “just deal with it.”

What Is Pelvic Floor Therapy?

Pelvic floor therapy (PFT) is a specialized form of physical therapy that focuses on the muscles, connective tissue, and neurovascular structures of the pelvic region. Your pelvic floor is a group of muscles that sit at the base of your pelvis, supporting your bladder, bowel, and uterus. When these muscles are too tight, too weak, uncoordinated, or impacted by scar tissue or hormonal changes, the effects ripple throughout your entire body.

A pelvic floor physical therapist uses hands-on manual therapy, movement-based techniques, dry needling, and exercise to help restore function, not just manage symptoms.

Who Is Pelvic Floor Therapy For?

The "I've Never Been Pregnant" Patient

One of the most common things I hear from new patients is: "I wasn't sure I qualified for this." You do. Pelvic floor dysfunction doesn't require a pregnancy history. Many women who have never been pregnant experience real, significant symptoms that pelvic floor therapy directly addresses, including:

- Chronic hip, low back, abdominal, or pelvic pain that hasn't responded to traditional physical therapy

- Bladder dysfunction: urinary urgency, frequency, incontinence, or recurrent UTIs and bacterial vaginosis

- Bowel dysfunction: constipation, hemorrhoids, incomplete emptying, anal fissures

- Pain with intimacy, inserting a tampon, or during a gynecological exam

These are not things you should have to normalize or push through. They are signs that your pelvic floor needs support and there is an evidence-based path forward.

The Perinatal Season: Fertility Through Postpartum

For women navigating fertility, pregnancy, and the postpartum period, pelvic floor therapy is one of the most valuable investments you can make in your body and your birth experience.

Fertility optimization is an often-overlooked application of pelvic floor PT. When the muscles and connective tissue of the pelvis are functioning well, it supports healthy circulation, reduces tension that can interfere with reproductive function, and helps prepare your body for pregnancy.

During pregnancy, pelvic floor therapy can reduce pain, support posture and pelvic stability, and help prepare your body for labor and delivery including techniques to reduce the risk of tearing and support an effective, coordinated push.

And postpartum recovery? This is where I want to challenge a deeply ingrained misconception: the postpartum season is not three months. It is 12 months and often beyond. Whether you had a vaginal delivery or a cesarean section, your body went through a huge physiological event. Scar tissue, core dysfunction, pelvic floor weakness or tension, and musculoskeletal imbalances don't resolve on their own by week six. A structured, personalized return-to-activity plan including return to running, lifting, and high-impact exercise is something every postpartum woman deserves.

Hormone Transitions: Endometriosis, Perimenopause, and Menopause

Hormones and pelvic floor health are deeply interconnected, and this is an area where so many women are underserved.

For women living with endometriosis or adenomyosis, pelvic floor therapy plays a critical role both before and after surgical intervention. Chronic pelvic pain, painful periods, and the effects of repeated inflammation and adhesions create compensatory tension patterns throughout the pelvic floor and surrounding tissue. Pre-operative pelvic PT helps prepare your body, while post-operative care supports healing, scar tissue management, and a return to functional movement.

As women move through perimenopause, fluctuating estrogen levels begin to affect the pelvic floor contributing to urinary changes, pelvic heaviness or prolapse symptoms, and discomfort with intimacy. This transition can span a decade, and having support during it changes everything.

In menopause, the sustained drop in estrogen leads to changes in tissue elasticity, vaginal dryness, and pelvic floor muscle tone that can cause or worsen symptoms many women assume are simply inevitable. They are not. Pelvic floor therapy, combined with a treatment plan personalized to where you are hormonally, can meaningfully improve your quality of life.
